• Full name, cell phone number & email address of each person desiring to receive services

• The type of service(s) each person desires to receive

• The length (timeframe) of each  service desired

• The date and time you desire services (providing multiple options is best)

• Clarification if you have a mobile or POP UP SPA DAY request (include address if mobile)

PLEASE NOTE: It is preferred that every person have a separate email for their email confirmations to be sent. This allows each client to complete all intake forms, have access to their own login account and be able to purchase product, cancel, reschedule and book services. One can also add others to have the ability to book on their behalf. If you are surprising a client by gifting their with a service or gift certificate, you can opt for the client to not be contacted or you be cc'd on their correspondence.
